Chronic Disease Management and Prevention 
The Division provides resources, education, training and support to GPs and practice staff to access and utilise Medicare Benefits Schedule (MBS) items and Practice Incentive Program (PIP) Service Incentive Payments (SIP) relating to the prevention and management of chronic disease.
Since their introduction, these items have undergone significant changes and have been joined by other items and service incentive payments (SIPs) which focus on prevention and management strategies.

- Home & Residential Medication Management Reviews
- Diabetes & Asthma Annual Cycles of Care (SIP)
- Mental Health Treatment Plans
- Case Conferences
- GP Management Plans
- Team Care Arrangements
- Comprehensive Medical Assessment (Residential Aged Care Facilities)
- Contributions to Discharge/Care Plans
- Aged Health Assessment (>75 years/ATSI >55 years)
- ATSI Health Check 15-54 years
- ATSI Child Health Check 0-14 years
- Healthy Kids Check (4 years)
- Refugee Health Check
- Intellectual Disability Health Assessment
- Diabetes Risk Assessment (40-49years)
